Well, it seems to a self-extracting archive. So just let it extract somewhere where you can find it again, the find the folder with the portraits in them (lots of .bmp files), and move those to your [path]\Baldur's Gate\portraits folder. Any bmp files of the correct size and format you should be able to see once you create a new character and select the custom portraits option.

    From the readme:
Though the small one may be a 24-bit bitmap file too.

Create the folder then. Right click and hit creat folder and name it Portraits

Also the sizes have changed for the EE version they are now larger then the old style

Here are the new sizes to be used with EE

Large - 210x300 (not required - character creation screen)
Medium - 169x266 (for the character record screen)
Small - 54x84 (for the right sidebar of the game screen)

This thread will help as well

One of the patches made it so it's not required to have the small medium and large portraits. If you place a portrait in the folder, the game will automatically resize it for you.

I'm fairly sure you can change portraits in Shadowkeeper in an on-going game as well. Only fairly sure because I don't have the EE version, in the original versions (of BG and shadowkeeper) I know you can do this.

I've been having trouble with this as well. When I try to extract, it doesn't give me any .bmp files. It just gave me another archive:
