I keep seeing things that tell me the TYPES of diseases but nothing telling me how you go about figuring out which one you use.
For this I'm trying to figure out how to determine which disease takes effect from the Contagion spell. Do you get to choose?

    What edition is this ???

Assuming 3.5 there's a table for that. The GM picks/rolls for the disease that contagion afflicts the target with if they fail their fortitude save.
Contagion specifically has 7 possible diseases it can inflict : http://www.dandwiki.com/wiki/SRD:Contagion
5 more can be found over in the table here : http://www.dandwiki.com/wiki/SRD:Disease

If we're talking 5e, it's simplified a lot, with the dm straight up getting one of 5 choices as to the disease effect upon a successful spell attack : http://thebombzen.github.io/grimoire/spells/contagion

Yes 3.5 sorry. I always forget to mention 3.5

How do you "roll" for it? There's 7? If you use the added 5 that would be a simple roll of a d12.
You should get to pick which one if your the caster no??

Normally the caster gets to choose (DM chooses in case an npc/enemy causes the effect.) i have had instances where the DM would roll for it though.
You can roll a d10+d4 then take the mean (i.e rolling a 1 or 2 is 1st result on table, rolling 3 or 4 is second result, etc.)

The caster chooses which disease the target will be struck with.

As a side note, never use two dice to roll for uneven dice numbers, it'll skewer the results. For a d7, simply roll a d8 and reroll any 8s. In case of d3 and d5, you can use d6 and d10 respectively and cut the result in half. If you keep having to roll non-standard dice, I suggest using a dice roller.
