My main character's face is gone (graphical error)

    I have a graphical problem (I tried the search function but I couldn't find any references to this yet).
    Somewhere along the game (not right from the start), if I remove my main character's helmet (she is wearing the Watchman's Helmet, but it remains the same with the paladin's helmet), her whole head looks like she is wearing a closed, rusty helmet that covers her whole head (which looks rather ugly). If I put the helmet back on, I see the helmet graphics with her face underneath again.

    Does anyone know how I can fix this?
