Hopefully this hasn't been done yet and is O.k. to put here.I finally found one of these blacksmith dudes and got sucked into seeing what every item on the list was like. Hope this helps somebody... :)
All of this done in Chapter 4 My fighter was 12th or 13th level when I made everything so if their isn't a level requirement listed it's obviosly at or below 12/13.

Notes for better use of formulas:
Armor:
If it says Ac Modifier 6 then you need a piece of armor that has an Ac value of 6. That means you must combine the Base Ac modifier to the Magical Ac Modifier.
Weapons:
All weapons must be magical to start. The kicker is that it doesn’t matter how magical they are. To make the Astral Blade you need a diamond and the cash and ANY magical longsword. It doesn’t matter if it’s +3 or +1 or if it has other magical properties. Once diamond, cash, and item are combined it will become the new item. You lose all previous properties of the original item. If you used a +3 Longsword in the combo, it would become a +2 Longsword.
Side notes:
You can only make one of each item.
I don’t know why the Foundation Light Flail is valued at 0 gold. Maybe its part of a quest or just a bug.
