There hasn't been an official release date set yet as far as I'm aware...

This winter at worst, hopefully.

I believe they're going to try to release it everywhere at the same time.

The application forms people are talking about are at ALFA (http://www.alandfaraway.net) an online persistent FR world being built for NWN.
