I just finished for the first time. It was lovely to win and all but when I clicked on the eyes to open the door to Irenicus, my alignment changed from Chaotic Good to Neutral Evil.
Does this happen to everyone?
Just wondering because I have a couple other games going and I don't want that to happen again. I want to take a good character to ToB.

In each of the 5 hell trials there is a "good" choice and an "evil" choice. If you've chosen even one evil path your alignment will be altered to NE. If you wanna return to CG you'll either have to load an earlier save and be more careful in your descisions or you can use ShadowKeeper and delete anything that says something like "alignment change" under affects. Do note that the rewards from using the tears are different for the good and evil choices so using SK to keep your alignment Good but recieving the evil rewards would be cheating.

Trial 5: Wrath

Good Path: Kill Sarevok without giving into hate.
Reward: +1 to Wis and Cha

Evil Path: Kill Sarevok out of hate.
Reward: +2 to Str

Hmmm...
I got +1 to Charisma, so it couldn't have been Sarevok.
Maybe it was the dragon. Seems like I read somewhere that you get a chance to speak to him, but he was red when he appeared so I must have messed up something when I spoke to that demon.

It was probably the dragon one.

The others are keeping Blackrazor, accepting the Nymphs Skin cloak, or sacrificing a party member.

Good Path: If you question pride, then you get the Silver Dragon.
Reward: +20% resistance to fire/cold/electric damage

Evil Path: If you charge into the room intent on killing everything you get the Red Dragon.
Reward: 200,000 XP

Easy choice here even if you are evil! :D
