Is Sarevok's alignment change part of the standard TOB plot or is it a mod?

Will Viconia's alignment change occur if I have the concurrent romances installed?

Sarevoks alignment will change also in the original game (without mods) if you choose the right conversation options and do the right things

Viconia's alignment will change if you have concurrent romances. You just have to make sure that you choose the right dialogue options.

If I go to Saradush, I can't get Viconia's alignment to change. What do I need to do? I've tried using Shadowkeeper to adjust the
Viconiaromance variable, but it does NOTHING :bang:
Viconiaromanceactive is currently set at 2

Don't bother with it; it's nothing special and doesn't even fit to the theme. Simply take SK and set her alignment to "chaotic good" -makes it a whole lot easier. That is, at least, what I do when I start a new game. ;)
