There's a fairly huge difference between "1st person" and "top-down" type cameras in the way you go about optomizing a graphics engine. Given that and the nature of NWN2's gameplay it makes the most sense to make the top-down view a priority.
No, there's no great technical hurdle to simply move the camera anywhere you want... it wouldnt take much work... but thats not the issue. The real issue is quality... the quality of the gameplay and of the presentation. To maintain the quality of the game in two disparate views is a lot of work... enough work that some other part of the game would likely suffer for it.
