I've added quite a few add-ons, including custom templates, custom items, removed exp-cap, multiple strongholds, etc.
When I upgrade to Throne of Bhaal, will these things simply stop working, or am I going to have to start a new saved game?

Your custom items should work just fine if they are in override, which is only way to get them work. BIF...
